Add source-preserving theme documents

Keep editable theme state in C so menu scripts can preview and modify
themes without receiving filesystem capabilities. Preserve original INI
lines and record field-level history; grouped imports retain every style
and glyph section, including custom definitions and unknown fields.

Validate saves through the existing loader, detect external changes, and atomically replace theme files. Reload the active theme only when its
selected file was changed.

Expose the document through the menu VM and cover editing, persistence, conflict handling, source preservation, imports, and undo/redo in the C
test suite.
